How do you get proof or evidence that you have received your 1^st dose of the COVID-19 vaccination?

Your proof or evidence can include any of the following: Letter from Dr or medical centre / clinic or pharmacy. Email confirmation or photo of text message of attended appointment booking, Your immunisation history statement available after your 1st dose on your  Medicare online account or Medicare app through myGov (for a step by step guide on how to get your immunisation statement click here ) or your Covid-19 digital certificate also available through your Medicare online account or Medicare app through myGov (available after your 2^nd dose)

What you need to do to receive your $100 e-gift card

(complete once you have received your 1st dose of your vaccination)

• Fill in the form entitled "LHF $100 Covid-19 e-Gift card incentive form"
• Attach a copy of your evidence / proof
• Email completed form along with a copy of your evidence / proof to [email protected]. Alternatively give to your Team Leader (they will pass it onto the appropriate person)
• Once processed Simone will make appropriate arrangements for you to pick up your $100 gift card. As you will be required to sign for it (please allow up to 14 days for this process)
